I have resolved this issue by re-saving every extension so this support message can be closed.

For some reason all of our extensions are showing up as busy (when they"re not). It"s possible to dial out, but it"s not possible to dial any other extension.
Incoming calls to a group or queue aren"t working either as every extension is shown as "busy".
