Error Too many connectins

I have a user "megasystem" on a premium account that has reported the following error to me , but he has not told me where it has come from

[nativecode=Too many connections] ** mysql://asteriskuser:amp109@localhost/asterisk

He also told me at 3:30 AM Eastern Time , June 26th that the PBX had been down for over 30 minutes
