No valid extensions in Digital Receptionist

I've setup a Digital Receptionist, and it worked great for a while. Now for some reason no matter what numbers callers press it just says "I'm sorry, that is not a valid extension." My inbound truck is using Google Voice.

Any ideas on how to fix this?

UPDATE: Only the number 3 works
